8/13/2023 EnchantaVaughn Approved Behavior: Upon arrival, Laylah was observed helping her mom with different chores around the house (i.e. washing dishes, folding clothes, taking out the trash, and cleaning up her room). Laylah appeared genuinely happy and energetic when greeted by QMHP. Laylah was cheerful and expressed good manners. Laylah was patient and waited for QMHP to direct her through her treatment plan. Laylah was observed following directions and listening to her counselor. Behavior: Laylah transitioned into the next activity without conflict and proceeded to engage into meaningful conversation. Laylah expressed that she wanted to look into more activities that could benefit her and help her deal with her stress at home better. Laylah expressed positive thoughts about herself stating “I feel amazing”. Laylah verbalized, “I’m learning so much about myself” in response to the assignment. Laylah was able to stay focus during session. Intervention: QMHP conducted family session. QMHP debriefed and inquired about her daily experiences. QMHP supported Laylah making good decisions and having positive behaviors in the home. QMHP expressed gratitude toward positive interaction with mom and sibling. QMHP supported and monitored Laylah in social and academic setting to ensure she was gaining insight on her mental illness. QMHP commended Laylah on utilizing coping strategies to increase her understanding of feelings and emotions. QMHP provided guidance on alternative behaviors and clear instruction on the alternative behavior. QMHP provided positive feedback when Laylah engaged in appropriate behavior. QMHP encouraged Laylah to continue displaying positive behaviors. Laylah utilized relaxation strategies promptly. The Laylah showed her gratitude for being able to stay relaxed and calm throughout the session. Intervention: QMHP supported Laylah involvement in selected activities. QMHP processed with Laylah about what she learned from activity. QMHP praised Laylah for her cooperation. QMHP encouraged Laylah’s mother to provide frequent positive reinforcement to maintain Laylah's interest and motivation in maintaining positive behaviors and completing her “at home” school assignments and/or chores. QMHP praised Laylah for listening and following directions and interacting safely during each intervention. QMHP observed Laylah complying with rules and expectations. QMHP explored the positive behavior options she was exhibiting with Laylah to support her willingness to continue to make good decisions and have positive behaviors in at home, the program, and in the community. Laylah administered positive behavioral expectations, reducing ADHD and meeting objectives. Response: Laylah said she woke up feeling good and was ready to have a good day. Laylah expressed positive thoughts about herself. Laylah stated she didn't need help with anything at the moment but would say something if she needed help. Laylah communicated by nodding and exhibiting good eye contact. Laylah verbalized positive feelings. Laylah displayed respectful, non-aggressive behaviors in the home. Laylah was pleasant and cooperative and demonstrated a willingness to discuss the factors contributing to her desirable mood. Laylah was able to process making good choices with QMHP and her mom. Laylah listed 2-3 behaviors that she was going to utilize in efforts to remain positive in the home. Laylah appeared to be receptive to receiving positive reinforcement from her mother, as demonstrated by her smiling and warm inviting body language. The frequency and intensity of the Laylah's defiant affect decreased. Response: Laylah displayed good participation and was responsive to all instructions, interacting well with mom and QMHP. Laylah responded in a positive manner acknowledging the positive reinforcement that the QMHP provided. Laylah responded well to feedback and discussed some things she can do better when it comes to new challenges. Laylah acknowledged interventions and verbalized clear understanding of each intervention. Laylah began to show greater control of her anger. Laylah appeared to decrease the intensity of her anger and frustration. Laylah was able to detect and acknowledge both positive and negative responses modeled by QMHP. Laylah is learning to calm down, think first, and respond without anger and frustration. Laylah has implemented the correct behaviors, by avoiding disruptive and impulsive acts. Laylah remembered to apply herself and utilize her strengths to focus on her treatment.
